Program Overview

With the acceleration of the global population aging process, the health issues of the elderly have become a major challenge urgently needing to be addressed in the field of public health. The elderly population in our country continues to grow, and health problems such as frailty and falls have gradually become core factors affecting the quality of life of the elderly. Early screening and intervention for frailty and falls have been universally recognized as important strategies to reduce the risk of falls and improve the overall health level of the elderly. By using assessment techniques for elderly frailty and falls to identify high-risk individuals early and taking targeted intervention measures in a timely manner, it helps to reduce the occurrence and progression of frailty, decrease fall-related complications, and thereby improve the quality of life of the elderly population. The management of frailty and falls is not only a focus of clinical care but also an important topic in geriatric medical research. Therefore, enhancing the professional capabilities of medical staff in comprehensive assessment techniques for frailty and falls is of great significance for upgrading the overall level of elderly care disciplines and optimizing the health management of the elderly population.

1. A Multidisciplinary Teaching Team

The elderly multidisciplinary team is a specialized group that provides comprehensive medical, rehabilitation, and nursing services for elderly patients. It comprises geriatricians, rehabilitation therapists, nurses, psychologists, nutritionists, pharmacists, and others. By integrating medical resources and formulating short-term and long-term treatment plans, the team helps elderly patients recover functions, stabilize their conditions, reduce complications, and thereby enhance their quality of life.


2. Standardized Operational Procedures

Strict adherence to the assessment technical processes for elderly frailty and falls, as well as the standardized implementation processes for elderly patient nutrition, which are interrelated and complementary, jointly ensure the health and quality of life for the elderly.


3. Advanced Assessment Techniques for Elderly Falls and Frailty

Advanced assessment techniques and methods provide more accurate and comprehensive means for evaluating elderly falls and frailty. They facilitate the development of personalized treatment plans, improve medical efficiency, prevent potential health issues, reduce medical costs, enhance the well-being of the elderly, and strengthen health management awareness.


4 Immersive Aging Experience and Empathy Education

From the beginning of the training, participants are organized to undergo an immersive simulation of the "aging" experience, personally experiencing the physical and mental changes caused by aging. This aims to increase participants' understanding of aging and interest in elderly care, better comprehend the needs of the elderly and their care, and enhance participants' understanding and empathy for the lives of the elderly.


The Department of Geriatrics at the First Affiliated Hospital, Zhejiang University School of Medicine possesses comprehensive teaching facilities and a humanistic care system. Based on the concept of "whole-person, whole-process service," the nursing team consistently puts patients at the center, innovates continuously, and strives for excellence.

Training Objectives

Our plan aims to prepare participants to implement a comprehensive multidisciplinary team management strategy for frailty and falls in elderly patients. Through a series of theoretical training, practical skills training, and case management models, it is designed to enhance participants' professional knowledge and practical skills in the field of elderly care, thereby providing safer and more effective nursing services for elderly patients.

The program offers a two-week full-time training experience.

Nurse:

· College nurse works for 5 years or more

· Bachelor degree nurse works for 3 years or more

Comprehensive Management of Elderly Frailty Based on the OEC Model

Skills:

· Elderly Fall and Frailty Assessment Techniques:

· Bioelectrical Impedance Analysis (BIA): Assessing body composition by measuring muscle mass and body fat levels in patients.

· Five Times Sit-to-Stand Test: Primarily used to check the lower limb strength of patients.

· Timed Up and Go (TUG) Test and 4-Meter Gait Speed Test: These tests are designed to assess the balance and walking ability required for daily activities in elderly patients.

· Grip Strength Test: Evaluating the muscle strength of the upper limbs.

· Static Balance Test: Specifically assessing the balance ability of the elderly.

· Reaction Time Test: Assessing the reaction ability of the elderly.
