Program Overview

Background: Oral carcinoma ranks eighth in the incidence of systemic malignant tumors, and its incidence has increased year by year in recent years. In the whole Zhejiang Province, there are about 1500-2000 new cases of oral carcinoma every year, of which about 1/5 patients are treated in our hospital. It is one kind of the key diseases and dominant diseases in our department. Oral and maxillofacial region plays an important role in language, chewing, swallowing and appearance. Oral carcinoma has caused functional and appearance damage to patients, causing great trauma to their physiology and psychology. Thus, the majority of patients are eager to treat it. Patients have an increasingly urgent need for the first-stage repair and reconstruction after oral carcinoma resection, and the need for minimally invasive treatment and functional recovery of oral carcinoma is also growing year by year.


Purpose: To meet the need for minimally invasive treatment and functional recovery of oral carcinoma, we conduct the comprehensive treatment and digital functional reconstruction of oral carcinoma.

Program Highlights

Our department has more than 30 years of experience in the treatment of oral carcinoma. It is a national clinical key specialty and a key medical discipline of Zhejiang Province. At present, a multidisciplinary and comprehensive diagnosis and treatment innovation team for oral carcinoma has been established, which has the advantages of multidisciplinary talents, advanced comprehensive diagnosis and treatment concept and repair and reconstruction technology, and sufficient clinical cases and biological specimen library. Our oral cancer team receives an average of 350 inpatient cases of oral cancer every year. In the past five years (2019-2023), our department has carried out extensive resection of large-scale oral carcinoma and over 500 cases of soft and hard tissue defect repair and reconstruction during the same period. Applications include forearm free flap, anterolateral thigh flap, vascularized fibula flap, blood vessel flap and non-vascularized iliac flap and other soft and hard tissue flap. Among them, the technology of digitally designed vascularized fibula flap and one-layer half-fibula flap with nail channel transfer is the first new technology in China, and its results were published in the top journal of Oral Oncology. The results of mandibular defect repair won the second prize of Science and Technology Progress Award of Zhejiang Province (Clinical Innovative Technology of mandibular functional Reconstruction and related basic research). Our department is also the first hospital in the province to carry out navigation verification and immediate intraoperative fibula flap implantation during the repair of soft and hard tissue defects in oral and maxillofacial region. At present, the oral carcinoma team of our department is actively exploring and carrying out clinical trials to study the effect of preoperative neoadjuvant therapy on improving the treatment effect of oral cancer and reducing the damage of surgery on patients.

Prospected Outcome:

1. Professional Skill Enhancement: Participants will master the latest techniques and methods in oral cancer comprehensive treatment including surgery, radiotherapy, chemotherapy ,and digital functional reconstruction.

2. Research Collaboration Promotion: Establish an international research collaboration network through the workshop to jointly conduct clinical research and technological innovation.

 

Goal of the Program:

1. Network Building: Establish a sustainable international exchange network to provide participants with a platform for subsequent cooperation and resource sharing.

2. By achieving these objectives, the project aims to contribute to the development of the global oral cancer treatment field and enhance the hospital's position in international medical cooperation.

Program Duration

The program provides a full-time, 1-month to 3-month training experience. In special circumstances, arrangements can be made for other lengths of training depending on your needs and interests.

Target Audience & Qualifications

• Fellows: Must have completed a general surgery residency program at an accredited institution.

• Residents: To be eligible for the Stomatology residency, you must have successfully completed a board-approved program in general surgery. Your general surgery program may be completed either through Mayo Clinic School of Graduate Medical Education or another accredited program.

• Students:

1. Must be a final-year medical student when you are scheduled to participate in the rotation. This means all your core/required clerkship rotations are complete at the time of the clinical elective.

2. Student majors in oral and maxillofacial surgery or Dentistry.

1st Week: introduction & welcome, program overview and objectives

Weekly: preoperative preparation, surgery planning, digital design

 

Tuesday

Weekly: outpatient observation, academic lecture, basic surgical skills training

Biweekly: mentor one-on-one training, transplant surgery observation

Weekly: MDT conferences, complicated case study, clinical rounds

Biweekly: simulated surgery training

Weekly: surgery hands-on

Weekly: complicated case study, comprehensive therapy

Last Week: wrap-up & reflection, participant presentations or discussion of key takeaways, feedback session (e.g. online survey or live Q&A)

Evaluation and Feedback

Monthly, an aggregate evaluation of a fellow's clinical progress is conducted through a clinical competency faculty committee. In last week, a final test will be hold.

Participants can provide feedback by instant messaging software.
